These duties may be performed after specific prior discussion with the supervising physician. The CRNA works with an anesthesiologist in the performance of duties. These duties include but are not restricted to: Performing and documenting a preanesthetic assessment and evaluation of the patient, including requesting consultations and diagnostic studies Performing a thorough check out of equipment. Selecting preanesthetic medications and fluids. Developing and implementing an anesthetic plan. Initiating the anesthetic technique, which may include general, regional, or local anesthesia, as well as monitored anesthesia care (MAC). Selecting, applying and inserting appropriate non-invasive monitoring devices for continuous evaluation of patient physical status in ASA 1,2,3,4,and 5 patients. Selecting, obtaining and administering anesthetics and adjuvant drugs and fluids necessary to manage the anesthetic care of the patient. Managing a patients airway and ventilation using current practice modalities. Facilitating emergence and recovery from anesthesia by selecting, administering, and ordering medications, fluids, and ventilatory support. Providing post-anesthetic follow-up evaluation and care. NON-ROUTINE DUTIES Monitoring pain management postoperatively using a numerical pain rating scale Responding to emergency situations by providing airway management for emergency life support techniques. EMERGENCY DUTIESThe CRNA is authorized to perform emergency duties for patients with life-threatening situations where a physician is not immediately available. The CRNA initiates these activities and makes every effort to summon a physician as soon as possible. Examples of emergency duties include but are not necessarily limited to the following: Cardiopulmonary resuscitation (CPR) Tracheal intubation and management of airway emergencies Advanced cardiac life support ROLE RESPONSIBILITIES AND ACCOUNTABILITIES Role responsibilities and accountabilities include demonstration of knowledge and skills necessary to provide care appropriate to the age related needs of patients served in the assigned area(s) of practice. This knowledge includes undertaking changes associated with aging and principles of growth and development relevant to the Adult and Geriatric population.In addition, the CRNA is accountable and responsible for possessing knowledge and skills to: Communicate and interact appropriately and courteously with all internal and external customers. Maintain confidentiality of electronic, written and/or verbal patient/employee information Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment and adherence to safety practices. Comply with all medical center and service/service line safety policies, procedures and protocols to avoid injury and/or illnesses to self and others. Identify and report all unsafe conditions to appropriate personnel to ensure a safe environment for all patients, medical center personnel and visitors. Practice safe working habits to avoid damage to equipment and building structures. TOURS OF DUTY 0730-1600 is the most frequently used tour. However, occasionally 0930-1800 may be the required tour. Additional tours of duty may include 0730-1800, 0930-2000, 1130-2000, when the DVAMC expands operating room capability and as determined by departmental needs. If on call hours are required for Monday through Friday, call hours will be from 2000-0730. If on call hours are required for Weekends, call hours will be 0730-0730 for 24 hours Saturday or Sunday. Needs for CRNA contract coverage are usually 1-3 days and occasionally a full week. Needs are driven by the number of residents scheduled and the number of staff CRNAs who are employed. There is the occasional need for multiple providers in the same week. QUALIFICATIONS Healthcare Providers assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Health Care Provider must meet all credentialing requirements of VA and pass evaluation and recommendation for credentialing and privileging by Executive Committee of the Medical Staff. Qualified personnel must be American citizens and be proficient in the English language. The qualifications of such personnel shall also be subject to review by the VA Medical Center, Chief of Staff and approval by the VA Medical Center Director. Applicants must provide evidence of at least one full, active, current and unrestricted licensure, registration, certification, and/or other relevant credentials, for verification prior to appointment and throughout the employment process, as requested. They must agree to accept the professional obligations delineated in the Medical Staff Bylaws, Rules and Regulations provided to them. They are responsible for keeping the Department of Veterans Affairs appraised of anything that would adversely affect or otherwise limit their clinical privileges. Note: Failure to keep VA fully informed on these matters may result in administrative or disciplinary action.All CRNAs will be required to enter all reports and medical treatment orders for incorporation into the computer patient record system (VistA/CPRS) on the day of the visit and satisfy VHA and medical center policies and JCAHO requirement for documentation. CRNAs must possess computer skills to access and input typed data on each patient into the computer patient record system (VistA/CPRS) utilized by the VA. Each applicant possessing a DEA certificate will document this information on the appropriate VA employment form, information about the current or most recent DEA certificate. Any applicant whose DEA certification (Federal and/or State) has ever been revoked, suspended, limited, restricted in any way, or voluntarily or involuntarily relinquished, or not renewed, will be required to furnish a written explanation at the time of filing the employment application and at the time of reappraisal. (If currently in VA applicants may waive DEA requirement if they already possess and active VA prescription number.) Please see above for requirements.
