Major Duties

JOB SUMMARY : Performs the primary function of the Registered Nurse by assessing, planning, implementing and evaluating the care of all assigned patients in the Post-Anesthesia and Ambulatory Surgical Unit. Is responsible for meeting the established ASPAN standards during the reacting of patients from an anesthetic and rendering post-operative care in the step-down ambulatory area, for managing all assigned personnel, supplies and equipment in PACU and the Ambulatory area, and for promoting team work with physicians and personnel in other departments. The Registered Nurse must exercise professional assessment and clinical skills as well as independent judgment to ensure the safety of the patient for both inpatient and outpatient/ambulatory patients. Requires strict adherence to standard precautions. Work hours will normally be 1 st shift, with on-call hours, as scheduled. Extended works hours may be required. Must assume the fi nancial responsibility to utilize time, equipment and materials with minimum waste.

EDUCATION, TRAINING, AND EXPERIENCE:
Education: Graduate from an accredited school of nursing with current licensure as a Registered Nurse in the State of North Carolina. ACLS and PALS certification required within one year of employment in the PACU area.
Training: Job requires strict adherence to standard precautions. Specific orientation to PACU and Ambulatory Nursing; involves a working knowledge of PACU/Ambulatory Surgical Unit techniques and procedures, and of surgical procedures, supplies and equipment employed in standard operations. Knowledge of the stages of anesthesia and of the typical reactions to the anesthetic agents. Thorough knowledge of the procedures and methods used in reacting patients and the post-operative nursing care of all types of patients.
Experience: Two years experience in the PACU/Ambulatory Surgical Unit preferred . Previous critical care nursing experience preferred. Must be familiar with the basic nursing performance outlined in the ASPAN standards.

ESSENTIAL FUNCTIONS :
Utilizes the nursing process in the assessment of patients ranging in age for infancy to the elderly to include, but not limited to: and individualized nursing care to patients and families. Sets priorities for appropriate nursing actions.
Develops appropriate goals for the post-anesthetized patient to include safety, instructions,
Initiates and implements physician’s orders.
Requisitions ordered diagnostic and therapeutic services.
Respects patient’s right to privacy.
Evaluates the patient’s response and outcome of nursing care.
Recognizes significant abnormalities in the pre and post-anesthesia state (ie: EKG rhythms, malignant hyperthermia, etc) and reports findings to the anesthesiologist and/or operating practitioner; takes appro priate nursing measures.
Provides nursing care for patients receiving moderate sedation (ie: Endoscopy care and management).
Adheres to universal protocol-confirms surgical site and surgical procedure.
Notes level of consciousness, laboratory values, allergies, etc.
Reduces anxiety of patients through explanation of actions taken and procedures done, with a calm and reassuring manner.
Exhibits knowledge in performance of Accucheck, peripheral blood samples, Mallampati assessment, and airway management.
Functions as PACU Registered Nurse during the recovery period: assumes availability of proper equipment for the patient. when indicated.
Receives report from the Operating Room Nurse, CRNA and/or Anesthesiologist and
Performs and charts patient’s assessment at fifteen (15) minute intervals or more frequently,
Ensures physician’s orders are implemented.
Monitors IVs, blood infusions, drains and drainage, Foleys, hemodynamic uni ts, epidural catheters, CVP and Art lines.
Assesses and implements discharge, using criteria for discharge from post-anesthesia care unit to ambulatory area or to the nursing unit.
Ensures patient is transported to surgical unit or ambulatory area; transfers to unit bed, when necessary.
Functions as Ambulatory Registered Nurse during the admissions and discharge period:
Receives outpatient on arrival, completes admission forms, obtains required signatures, consents, and releases, as needed and if not previously obtained.
Identifies patient, calls name, checks ID band with chart and operating room schedule.
Identifies self and indicates role to patient.
Reviews pre-operative checklist, insuring it is complete and pre-op procedures have been accomplished.
Prepares outpatient for examination, treatments and/or surgery; initiates IV fluids, performs required preps; assists with ESI procedures.
Ensures all physician’s orders are implemented.
Following procedure, receives patient from PACU or OR; completes initial patient evaluation and assessment, implements appropriate nursing interventions and evaluations.
Ensures patient is given postoperative discharge instructions.
Assesses and implements discharge criteria; ensures patient is transported safely from ambulatory unit to outpatient discharge area.
Assumes responsibility and supervises others in nursing activities related to the care of the patient in the Post Anesthesia Care Unit and Ambulatory Surgical Unit: provided by physicians to patient and family.
Hospital policies and procedures.
Acts as patient/family advocate in communications, aware of and reinforces information Cooperates with peers regarding workload.
Demonstrates willingness to work where needed; ie, holding area, eye preparation area, obstetrics for C-Section recovery, assisting with blocks, etc.
Takes fair share of high-risk surgical patients.
Demonstrates f lexibility with co-workers if work schedule altered; ie, call person reporting late, time schedule revision, call schedule revision, etc.
Maintains attendance and punctuality according to hospital and department policies.
Prepares and administers medications, IV fluids and blood, in accordance with OR/
Ensures Hand-Off communication for each patient is conducted to all team members.
Aids in general cleaning of area and cleans following own activities.
Observes constantly for any unsafe conditions occurring in the OR; intervenes immediately in known unsafe, illegal and unauthorized practices, when observed.
Completes Confidential Quality Improvement reports.
Participates in evaluating new equipment and supplies, as needed.
Knows location of emergency equipment, with working knowledge of such equipment, crash cart contents, defibrillator and documentation.
Acts rapidly and effectively, manages self, patient and other employees during any eme rgency situation.
Documents post anesthesia and ambulatory care properly, completely, legibly, concisely and promptly.
Participates in continuous quality improvement, attends Surgical Services/PACU/Ambulatory staff meetings and/or reads minutes form all meetings to remain informed. Attends Unit’s required inservice education programs. Attends all hospital mandatory classes.
Participates in educational programs to increase professional competency and meet personal needs and goals; assesses own needs for continuing education and suggest topics for programs.
Serves on hospital committees and/or Quality Improvement projects, as requested.
Participates in telephone call-backs for patient satisfaction.
Participates in developing, reviewing and revising policies and procedures as directed by the
Director of Surgical Services, Policy and Procedure Coordinator, and/or Unit Charge Nurse
Keeps current with all new and revised policies.
Interprets a nd implements procedures, policies and nursing care objectives to patients, families and peers.
Identifies learning needs of personnel and participates in teaching those needs.
Plans way to solve problems and to make improvements in the department, in collaboration with the Director of Surgical Services and/or Unit Charge Nurse.
Assists with orientation of new employees.

PHYSICAL REQUIREMENTS : Requires full physical capacity; the ability to withstand physical, mental and/or emotional stress; ability to stand or walk most of the day; ability to lift and/or carry (40) forty pounds, and to bend, kneel, reach or twist as needed. Requires the ability to perform on-call duty, as required. Exposure to disagreeable elements, not continuously present, including but not limited to dust, odors, blood, biologically contaminated items, sharp items, excrements and hazardous materials listed on MSDS.
MACHINE , TOOLS, AND EQUIPMENT: Must have working knowledge of IV pumps, glucometer devices, defibrillators, computers, EKG monitors, hemodynamic units, hospital beds, stretchers, calf compressions devices, etc., used in the PACU/Ambulatory area.
Must use care when operating, cleaning and storing all equipment and supplies.
